DD FORM 2734/1 THRU /5, COST PERFORMANCE REPORT, MS EXCEL FORMAT (MAR 2005)., This report consists of five formats containing data for measuring contractors' cost and schedule performance on Department of Defense (DoD) acquisition contracts, and is Ased in conjuction with DI-MGMT-81466A. ThE Microsoft Excel spreadsheet template (DD Form Number 2734/1, .2, /3, /4, & /5) is used in conjunction with DI-MGMT-81466A, COST PERFORMANCE REPORT (CPR). It consists of five formats containing data for measuring contractors' cost and schedule performance on Department of Defense (DoD) acquisition contracts. Format 1 (Sample Format 1) provides data to measure cost and schedule performance by product-oriented Work Breakdown Structure (WBS) elements, the hardware, software, and services the Government is buying. Format 2 (Sample Format 2) provides the same data by the contractor's organization (functional or Integrated Product Team (IPT) structure). Format 3 (Sample Format 3) provides the budget baseline plan against which performance is measured. Format 4 (Sample Format 4) provides staffing forecasts for correlation with the budget plan and cost estimates. Format 5 (Sample Format 5) is a narrative report used to explain significant cost and schedule variances and other identified contract problems and topics.
